We look forward to having you join our exercise class, but before you do … please read the disclaimer below:

I understand that I am responsible for monitoring my own health and condition during the Physical Exercise Class, and if any unusual symptoms occur I will cease to participate and if the symptoms persist, I will reach out for medical assistance. 

I agree to assume all risks associated with the Physical Exercise Class and hereby release and hold harmless Signature Retirement Living Corporation and all properties under their management, their directors, employees, independent contractors and agents who cannot be responsible for risks willingly assumed.  I therefore hereby release and forever discharge Signature Retirement Living Corporation and their properties under management, their directors, employees, independent contractors and agents from any health claims, suits, losses, all actions, damages, claims, and demands whatsoever arising by reason of participation in the Physical Exercise Class.
